Miss Sherina Balaratnam joined an international faculty at the BTL Face, Body and Mind Summit in Prague to share her approach to full 360 body treatment: a protocol that addresses the front, back, and pelvic floor together using Emsculpt Neo and Emsella.
The BTL Face, Body and Mind Summit brought together aesthetic medicine practitioners from across Europe and beyond to explore the science of ageing and the clinical protocols shaping the field. Miss Sherina Balaratnam, Medical Director of S-Thetics Clinic and BTL Aesthetics’ first non-US Global Key Opinion Leader appointed in over 30 years, was invited to present her approach to body ageing, sharing a protocol that goes significantly further than conventional body sculpting.
Her focus: the body does not age in isolation. The abdominal wall, the spinal structures, and the pelvic floor are all connected, and all change over time. Treating them together, in a single coordinated protocol, produces results that treating any one of them alone cannot.
A 360 Approach to Body Ageing
Most body sculpting protocols focus on one area: the abdomen. What is less commonly addressed is how the body ages as a system. Over time, the layers of the frontal abdominal wall can detach. The deep structures of the back weaken as postural habits change. And the pelvic floor, often overlooked entirely in aesthetic medicine, undergoes its own gradual deterioration.
Miss Balaratnam’s Core to Floor protocol treats all three. Using Emsculpt Neo for the core and back, and Emsella for the pelvic floor, the protocol addresses the full 360 of the body’s structural foundation.
“The body ages as a connected system. Front, back, and pelvic floor: all three need to be addressed together.”

Miss Balaratnam’s Role as a BTL Global Key Opinion Leader
Miss Balaratnam’s invitation to present at the Prague Summit reflects her standing as the first non-US Doctor to be appointed BTL Aesthetics Global Key Opinion Leader in more than 30 years. This is a role that places her at the centre of how BTL’s technology is understood, taught, and applied internationally.
With 26 years as a qualified Doctor, a seven-year background in NHS reconstructive plastic surgery, and 14 years specialising in aesthetic medicine, her clinical perspective brings a depth of anatomical knowledge that informs every protocol she develops. Her work with Emsculpt Neo and Emsella is an extension of this: not simply using the devices, but understanding precisely why they work and how they interact with the body’s deeper structures.
The Core to Floor Protocol: Emsculpt Neo and Emsella
The protocol combines two BTL technologies, each addressing a distinct part of the body’s structural foundation:
→ Emsculpt Neo: Uses a combination of radiofrequency and high-intensity focused electromagnetic energy to build and strengthen the muscles of the abdomen and back. It addresses the detachment and weakening of the frontal abdominal wall and deep spinal musculature, rebuilding structural support from within.
→ Emsella: Targets the pelvic floor using high-intensity focused electromagnetic technology to stimulate thousands of supramaximal pelvic floor muscle contractions per session. It completes the 360 approach by addressing the underside of the body’s core, an area that conventional body sculpting almost entirely overlooks.
The recommended course is four treatments over four weeks, followed by a maintenance and toning session every three months. Miss Balaratnam has personally followed this protocol for eight years and continues to feel the benefit: stronger, more lifted, and more structurally supported than before.
What This Means for Our Patients
Body ageing is rarely discussed in full. Patients often present with concerns about the abdomen, without knowing that the underlying cause may be structural: a weakening of the muscle layers rather than a surface-level issue. For patients who have noticed changes in posture, core strength, or the way their body feels and functions over time, this protocol addresses those changes at the level where they originate.
Pelvic floor health is also increasingly recognised as central to overall wellbeing, yet remains underdiscussed. Emsella brings clinical-grade pelvic floor treatment into the same conversation as aesthetic medicine, where it belongs.
If you would like to understand whether the Core to Floor protocol is appropriate for you, we would be glad to advise at a consultation.
